Short answer: Most scholars, including the Islamic Fiqh Academy, hold that non-nutritive injections under the skin or into the muscle do not break the fast, so a prescribed peptide pen can usually continue during Ramadan. The practical change is timing: most patients move their dose to after iftar or before suhoor. Weight-loss peptides need extra care because of dehydration and low blood sugar. Ask your physician to adjust your plan before Ramadan starts, and ask a trusted scholar if you are unsure about the religious ruling.

Key facts

  • Subcutaneous injections: considered by most contemporary scholars not to break the fast when they are not nutritional.
  • Nasal sprays: more debated. Many scholars allow them if nothing is swallowed, others advise using them after iftar.
  • Best dosing window: between iftar and suhoor, at the same time each night.
  • Highest-risk group: people on appetite-lowering peptides such as GLP-X3, and anyone with diabetes.
  • Storage: keep the pen in the fridge and out of a hot car during long Taraweeh and family visits.

Does a peptide injection break the fast?

In 1997 the Islamic Fiqh Academy of the OIC, meeting in Jeddah, listed injections into the skin, muscle or vein as things that do not invalidate the fast, with the exception of intravenous feeding. Peptide pens inject a very small volume under the skin and contain no food, so most scholars treat them the same way as insulin injections. Some people still prefer to inject after iftar to avoid any doubt, and that is a reasonable choice that rarely affects treatment.

This article explains the medical side. For the religious ruling that applies to you, ask a scholar you trust.

What about Semax and Selank nasal sprays?

Nasal sprays are different because some liquid can run to the back of the throat. The Fiqh Academy allowed nasal sprays when nothing is swallowed, but many people choose to use them after iftar. Both Semax and Selank are usually taken once or twice a day, so moving them to the evening is simple.

When should I take my peptide during Ramadan?

Type of peptide Usual advice in Ramadan
Recovery and skin peptides (BPC-157, GHK-Cu, stacks) Any fixed time between iftar and suhoor.
Growth hormone peptides (CJC-1295, Ipamorelin, Sermorelin) Before sleep, at least two hours after the last meal, as the release is stronger on an empty stomach.
Sleep peptides (DSIP) Before sleep. Ramadan sleep is shorter and broken, so tell your physician.
Energy peptides (NAD+, MOTS-c) After iftar or at suhoor, not late at night, as some people find them stimulating.
Weight-loss peptides (GLP-X3, AOD-9604) Only as your physician sets it. The dose may need to be lowered.

Whatever the peptide, pick one time and keep it for the whole month. Consistency matters more than the exact hour.

Weight-loss peptides and fasting: the main risks

GLP-X3 and similar peptides lower appetite and slow the stomach. Fasting already does both, so together they can lead to nausea, dehydration, dizziness and, in people on diabetes medicines, low blood sugar. Your physician may lower the dose, delay a dose increase until after Eid, or pause treatment for the month. Do not stop or change the dose on your own.

Break your fast if you feel shaky, confused, faint or have severe vomiting. Islam does not ask a sick person to harm themselves by fasting.

Tips for the month

  • Drink water steadily between iftar and suhoor, around 2 to 3 litres in total.
  • Set a phone reminder for your dose, as the daily routine changes a lot.
  • Keep the pen in the fridge door, not in the car or a bag left in the sun. Our guide on how to store a peptide pen in Saudi heat has more detail.
  • If you travel for Umrah, carry the pen in an insulated pouch with a copy of your prescription.
  • Book a short review with your physician one to two weeks before Ramadan.

Common questions

Can I start a new peptide during Ramadan?

You can, but many physicians prefer to start after Eid so that early side effects, such as nausea or injection-site redness, do not happen during a fasting day.

Will a missed dose ruin my results?

No. Take it at the next planned time and do not double the dose. One missed dose rarely makes a difference.

Does injecting during the day break the fast?

Most scholars say a non-nutritive injection does not. If you want to avoid any doubt, inject after iftar.

Can I use a peptide pen if I have diabetes and fast?

Only with your physician's plan. Diabetes, fasting and appetite-lowering peptides together raise the risk of low blood sugar.
